MRI, X-RAY AND ULTRASOUND REFERRALS IN SOUTH KENSINGTON
Most musculoskeletal problems are diagnosed clinically and don't need a scan. But some do — and when imaging is genuinely needed, waiting months for it holds up your treatment.
Physiome can refer you directly for MRI, X-ray and diagnostic ultrasound at partner imaging centres in London, without needing to go back to your GP first. Scans are carried out and reported by qualified radiographers and radiologists at the imaging centre.
All imaging referrals follow a physiotherapy assessment. We need to examine you before we can decide whether a scan is warranted, which type would answer the question, and what we'd do with the result. We refer where imaging will change your management — and if it won't, we'll tell you that too, because a scan that doesn't alter treatment costs you money and often causes unnecessary worry. WHEN IMAGING IS USEFUL
- Suspected fracture or bone stress injury
- Suspected significant ligament or tendon rupture
- Symptoms not improving as expected despite appropriate treatment
- Suspected meniscal or labral tears where surgery is being considered
- Assessing severity before an orthopaedic referral
- Nerve-related symptoms requiring investigation
- Guiding decisions about injection or surgery
- Clarifying a diagnosis where the clinical picture is genuinely unclear
- Suspected inflammatory or systemic conditions requiring further investigation
WHEN IMAGING ISN'T USEFUL
It's worth being direct about this, because scans are frequently requested when they shouldn't be.
- Imaging findings correlate poorly with symptoms. Disc bulges, degenerative changes, rotator cuff tears and meniscal wear are extremely common in people with no pain at all — they increase with age and are often incidental. A scan showing one of these doesn't necessarily explain your problem.
- Scanning too early can also make things worse. Being told you have degeneration or a tear, when it may have nothing to do with your symptoms, tends to make people move less, worry more and recover slower.
We recommend imaging when it will change what we do. Otherwise, treatment and reassessment is usually the better path.
TYPES OF IMAGING
MRI
Detailed images of soft tissue — ligaments, tendons, cartilage, discs, muscle and bone marrow. Best for suspected ligament tears, meniscal and labral injury, disc problems, bone stress injuries and tendon pathology. No radiation. Takes roughly twenty to forty-five minutes and requires lying still inside the scanner.
X-ray
Best for bone. Used for suspected fractures, assessing arthritis severity, alignment and bone abnormalities. Quick, widely available and inexpensive, but shows little soft tissue detail. Involves a small dose of radiation.
Diagnostic ultrasound
Real-time imaging of superficial soft tissue, particularly useful for tendons, bursae, muscle tears and shoulder problems. Dynamic — the joint can be imaged while moving, which MRI can't do. No radiation, and often the fastest and most cost-effective option for suitable conditions.
We'll advise which is appropriate for your problem. The right scan matters as much as whether to scan at all.
HOW THE PROCESS WORKS
- Assessment. Book a 30-minute physiotherapy appointment. We take a history, examine you and reach a working diagnosis. Imaging is only considered once there's a clinical reason for it.
- Discussion. We explain why we think a scan is indicated, which type, what it will and won't tell us, and what we'd do differently depending on the result. Costs are discussed upfront.
- Referral. We issue the referral during the same appointment. You book directly with the imaging centre at a time that suits you.
- The scan. Carried out and reported at the imaging centre by qualified radiographers and radiologists. Physiome does not perform or report scans.
- Results. Once your report comes through, we'll talk it through with you over the phone at no extra charge — no need to book another appointment just to hear what it says. We'll explain the findings in plain language, what they mean in the context of your symptoms and, importantly, what they don't mean.
- Next steps. If the report changes your treatment plan, we'll arrange a follow-up appointment. If it indicates you need an orthopaedic, rheumatology or other specialist opinion, we'll advise and, where appropriate, refer you on.
COSTS AND INSURANCE
- Imaging is paid for separately to your physiotherapy appointment and charged by the imaging centre, not by Physiome. Costs vary by modality and provider — ultrasound and X-ray are considerably cheaper than MRI.
- If you're claiming on private health insurance, contact your insurer before booking a scan. Most require pre-authorisation and some require a GP or consultant referral rather than a physiotherapy referral. Getting this confirmed first avoids an unexpected bill.
- We'll give you an indication of costs before referring.
WHAT TO EXPECT AT YOUR APPOINTMENT
- Imaging referral isn't a standalone booking. Every referral follows an assessment, so book a 30-minute physiotherapy appointment at Ethos Sports Centre, South Kensington.
- We'll take a history, examine you and reach a working diagnosis. If a scan is indicated we'll explain why, which type and what it should tell us, and issue the referral during the same appointment. If it isn't indicated, we'll explain that and start treatment instead.
- Bring any previous scan results, hospital letters or GP correspondence, and details of your insurance policy if you intend to claim.

- Can a physiotherapist refer me for a scan?
Yes. Physiotherapists can refer for imaging where it's within their scope of practice and there's an established arrangement with the imaging provider. - Do I need to see my GP first?
Not for a self-funded scan. If you're claiming on private health insurance, check your policy — some insurers require a GP or consultant referral. - Can I just book a referral without being assessed?
No. Referring for imaging without examining you first would mean guessing at which scan you need and why. Book a physiotherapy appointment and we'll assess and refer in the same session if it's warranted. - Do I have to book another appointment to get my results explained?
No. We'll go through the report with you over the phone at no extra charge. If the findings change your treatment plan, we'll then arrange a follow-up session. - How quickly can I be scanned?
